Insurance and Fees
Included in my services to you, I complete all insurance paperwork on your behalf. I am an in-network provider for some Idaho insurance policies, and will be able to collect only the copay once we have confirmed the details of your insurance coverage. To find out if I am in-network for your policy, please speak with myself or our receptionist.
Remember, most plans (but not all) do provide benefits when working with an out-of-network provider. In this case you may simply have to meet a different deductible, or will receive a 50% benefit instead of 70%, for example. For out-of-network coverage, or when out-of-state policies do not easily provide plan details to providers, the fees below are paid in full at the time of our meeting. In these cases, I will still work with you to ensure that you receive the maximum insurance benefit to which you are entitled, paid directly from your insurance carrier to you.
Credit card, debit card and cash payments are accepted, as are personal checks up to $150. Please speak with me personally if alternative financial arrangements would allow you to seek services with me. Because I am committed to you at the appointed time, I require 24 hours notice for cancellations.
- $125 per 50 minute session
- $140 for the Initial Diagnostic Consultation
- $579 for complete Brown Scales ADHD testing, incl. 3 page final report (no checks for this service)
- $275 for Fertility Pre-Treatment Evaluation, incl. 2 page recommendation report. (no checks)
- $475 for Fertility Pre-Treatment Evaluation, if using a surrogate carrier (which requires significantly more interviewing, and testing with the MMPI-2)
